Free Hearing Evaluations
The Welch-Schmidt Center for Communication Disorders is filling appointments for complete audiology evaluations. This service is FREE for faculty/staff/students or their dependents.
*If you have been seen in the clinic before, just phone 543-4993 to schedule your appointment. Identify yourself as a client who has been seen before and your appointment can be scheduled at that time.
*If you are new to the clinic, please complete the appropriate audiology case history form (Adult or Child) and submit to Martin 34. When we have received your case history form in the clinic, we will contact you to schedule your evaluation. The case history forms may be found at link below.
