Requirements for all students/employees
The UCM Board of Governors, per recommendation of the US Center for Disease Control (CDC) and the Missouri Department of Health and Senior Services, requires all students and employees to present documentation showing immunity to Measles, Mumps, and Rubella and demonstrate a negative history for active tuberculosis (a TB screening is required for everyone).
Requirements for all students living in University Housing
The State of Missouri requires that all students living in University Housing be informed about meningococcal disease, and about the availability of a vaccine that can lower your risk of contracting the disease.

Please fill out and print the Immunization Form (link is at the top of the page in PDF format). Be sure to attach copies of all required documents needed for enrollment/employment to the Immunization Form. Fax, Bring, or Mail to:

Failing to comply with the immunization policy will result in a hold for next semester enrollment or suspension of employment.
