How to Report an Accident/Injury

Employee - Work Related Injuries - Human Resources

Report all work related injuries immediately to your supervisor and UCM Human Resources (x4255). Please include details such as who, what, when, how and if any hazards were present.

If serious or life-threatening injury occurs, immediately contact Public Safety @ 660-543-4123. Be prepared to give specific incident details to the Dispatcher. Then contact supervisor and Human Resources.

NOTE -> If it is a work-related injury/accident, the State of Missouri Workers Compensation - Central Accident Reporting Office (CARO) must pre-authorize medical treatment - unless the injury/accident is a life-threatening emergency situation. Call CARO at 1-800-624-2354. CARO directs care and will not authorize treatment at the University Health Center. CARO reviews all claim documents to determine responsibility of claim.

 

NON-Employee Injuries -  Not Requiring Medical Attention

Please email details to the Environmental, Health & Safety (EHS) Manager. Include the following:

  • Name and 700# (if student)
  • When the injury occurred – specific date and time
  • Building, room#, or if outside – specific location
  • How and why the injury occurred
  • Body part injured
  • If a hazard was present, describe hazard and what needs to be done to eliminate or reduce hazard

This information will be used to help prevent future injuries

NON-Employee Injuries - Requiring Medical Attention or is Life-threatening

  1. Do Not Move the injured party unless their surroundings are dangerous.
  2. Call 911 immediately and request an ambulance. Be prepared to give specific information about your location (building & room #) and the name of patient.  Stay on phone until Dispatcher has all appropriate information.  NOTE -> 911 Operator will notify Public Safety if injury is on UCM Campus.
  3. Have someone meet the Emergency Responders (Fire / Ambulance) and guide them to the patient.
  4. UCM Dispatch will send a UCM Police Officer to assist until the ambulance arrives.

The University appreciates being made aware of accidents and Public Safety will involve our office of Environmental Health and Safety as appropriate. Any expenses for medical care should be referred to the student/visitor's own insurance. As a state agency, the University is not responsible for such expenses. Any questions in that regard are outside of the University's responsibility and can be directed to the State of Missouri's Office of Risk Management.
