Immunization Policy

Requirements for all students/employees

The UCM Board of Governors, per recommendation of the US Centers for Disease Control (CDC), requires all students and employees to present documentation showing immunity to Measles, Mumps, and Rubella and complete a Mandatory Immunization Screening form. A link to this form is located at the bottom of the page under instructions. Documentation for immunity to Measles, Mumps, and Rubella must consist of 2 doses of the vaccine being administerd or a titer (blood test) proving immunity to Measles, Mumps, and Rubella. You will not be required to get a TB test if none of the statements in Part II, Tuberculosis (TB) Screening, of the form apply to you. Simply mark that none of the statements apply to you and no further documentation will be needed. 

Requirements for all students about Meningococcal Disease and Immunization.

The Missouri Revised Statutes, section 167.638, states that each public institution of higher education shall provide a copy of the brochure to all students; and if the student is under 18 years of age, to the student's parent or guardian. The State of Missouri requires that all students living in University Housing must receive the meningococcal vaccine. Two doses of MCV4 are recommended for adolescents 11 through 18 years of age: the first dose at 11 or 12 years of age, with a booster dose at age 16. Exemptions are allowed for medical/religious reasons. If you qualify for an exemption, you must fill out an exemption form. Exemption forms are available at the Health Center.

Please submit the completed Mandatory Immunization Screening form (link below), and be sure to attach copies of all required documents needed for enrollment/employment. Documents can be hand carried, faxed, or e-mailed to:

Failing to comply with the immunization policy will result in a hold for next semester enrollment or suspension of employment.
